DENVER DEPARTMENT OF HUMAN SERVICES OUTREACH SERVICES Outreach Vision: To enroll individuals in Medical and Food assistance within the City and County of Denver by developing an integrated outreach model that promotes the programs within human services through the community. In addition, Denver Human Services’ SNAP into Health initiative will work with community partners to bring increased access to healthcare and nutrition to communities who need it most. Denver Human Services is providing a holistic approach to serving the communities’ needs, including healthy living, health coverage, and financial selfsufficiency Major Focus Areas: Increase access to public assistance programs Form strategic collaborations with community partners to provide holistic wrap around services Goals Strategies Education and Awareness: Strategies Provide Navigator training Provide education regarding access to healthy food, healthcare, Financial assistance. Promotion of Farmers Markets, and access to Snap redemption Provide additional resources to link clients to tools that promote long term self-sufficiency. Access and Enrollment Increase enrollment of those eligible for food, medical, and cash assistance programs. Partnering with Hunger Free to bring SNAP Application assistance into the communities Training members of the community to assist others with assistance program applications Collaboration and strategic partnerships with the community to provide resource information and wrap-around services. Increase the availability of access to healthy foods and SNAP Redemption at farmer’s markets Administer nutrition education, financial coaching, and other community services on location. Public assistance program applications: SNAP Benefits: Food assistance for individuals or families that meet program guidelines. Expedited processing is available within 7 days of application if you meet program requirements. If you are receiving three meals a day, you do not qualify for SNAP benefits. Aid to the Needy and Disabled (AND): Cash assistance program for those with a qualifying disability. Medicaid: Medical Assistance for Adults. Old Age Pension (OAP): Is a cash assistance program for clients 60+ years old, without any income. General Assistance Services: Pharmacy vouchers: One-time emergency pharmacy vouchers can be issued for mandatory prescriptions, if no other resource is available. The vouchers are issued to King Soopers pharmacies only. Narcotics are not eligible. RTD Bus tickets: If needed, clients may receive a one-way ticket if a service was provided by a GA/Outreach team member. Family Reunification: Greyhound bus tickets for homeless clients to be relocated with family, only if they meet eligibility requirements. Eviction assistance, Deposit, First Month’s Rent, Mortgage or Xcel assistance: Financial assistance is available to individuals and families who have a current verifiable emergency preventing them from paying their rent and are able to demonstrate the ability to maintain the unit. Emergency Shelter: Outreach staff will assist homeless families with securing an emergency bed or room through the various shelter programs. Respite Program: Respite beds are provided for homeless clients discharged from specific hospitals. Burial Program: Burial assistance is available to Denver residents that do not have sufficient funds for burial or cremation. Assistance is based on all resources and income. ABOUT GIVE Denver GIVE Denver is a program of Denver Human Services that leverages contributions, volunteers, and community partnerships to bring hope and happiness to foster children, the homeless, and the City’s most vulnerable communities. The GIVE Denver program has three main components: The GIVE Denver Center, located at the Richard T. Castro building at 1200 Federal Blvd. in Denver, uses volunteer resource navigators to connect underprivileged individuals and families in crisis to necessary essentials like diapers and wipes; shampoo, soap and other hygiene products; and children’s pajamas. Our resource navigators also refer GIVE Center clients to community-based service providers, including food pantries, clothing banks, utility assistance, rental assistance and other lifeline resources. GIVE Denver carries out three annual drives - the Spring Gift Basket drive, Adopt-A-Student School Supply drive, and Adopt-A-Family Holiday drives - to provide gifts and supplies to children and families receiving assistance from DHS. While DHS’ caseworkers and family advocates partner with the courts, schools, foster parents and other caretakers to ensure the safety and wellbeing of our clients, GIVE Denver’s drives provide an additional measure of comfort that may be overlooked during times of crisis. GIVE Denver supports events, campaigns and other efforts to prevent the abuse and exploitation of children, the elderly, and other vulnerable populations; and to commemorate milestones of children and families assisted by DHS, including family reunifications, adoptions, and other key moments. GIVE Denver partners with corporate and individual sponsors to make these efforts successful. How is GIVE Denver funded? GIVE Denver is supported solely through the use of volunteers and donations, with DHS & the City contributing the 1,280 square foot space and two staff members who oversee operations. Where GIVE Denver and what are the hours of operation? The GIVE Denver Center is located at Denver Human Services Castro Building at 1200 Federal Blvd. Denver, CO 80204. GIVE Denver Success Story The GIVE Denver Center’s volunteer resource navigators helped a grandmother with five grandchildren who, because of difficult life circumstances that forced the family to move from Florida, arrived in Colorado homeless and without resources. The GIVE Denver program immediately connected the family with referrals for housing assistance to move the family out of the car they had been sleeping in, food assistance to ensure the children received regular meals before federal benefits could begin, and helped the grandmother enroll her grandchildren in Denver Public Schools. Now the family is in permanent housing and the children are flourishing in school. Adult Protective Services D enver Adult Protective Services (APS) is a program of Denver Human Services that assists at-risk adults who have been mistreated by another person or suffer from self-neglect. An at-risk adult is anyone 18 years or older who is unable to care or advocate for themselves, or may have a disabling condition. The APS unit assesses cases and provides short-term and ongoing protection services for at-risk adults. Our services seek to improve the health, safety and welfare of our clients, and ensure they have food, shelter and other basic life necessities. We also help adults who lack the capacity to make or communicate responsible decisions about their own personal affairs.
