Transition of Care Notice
Dear Parent or Guardian:
Congratulations! Welcome to the Staywell Kids family. We look forward to serving your
child’s health care needs.
We want to be sure your child continues seeing his or her doctors and getting all the
right medications. Please have the doctor call our Intake Service Department 24 hours
a day, 7 days a week at 1-866-698-5437 if any of the following apply to your child:
• Takes regular medication(s) that need(s) authorization
• Sees a specialist
• Receives therapies (for example, occupational therapy or physical therapy)
• Uses durable medical equipment (for example, oxygen)
• Receives in-home services (for example, wound care or in-home infusion)
