ATTENDANCE CONTRACT
ATTENDANCE POLICY
The Hispanic Mother-Daughter Program requires that all parent and student participants adhere to the
workshop attendance policy. If a participant does not attend regularly, the student will not receive the full
benefit of the program. ASU and your respective districts invest time and money to allow students and their
parents to participate. Therefore, participants must share in the responsibility of making this program a
success.
The policy is as follows:
1) If either parent or student is absent from a workshop, it is considered an absence.
2) No other individual can take the place of the parent or participant without prior permission.
3) If participants arrive at the workshops after 7:00 p.m., they will be marked absent.
4) If either parent or student participants miss more than (1) workshop during the academic year, both
will be dropped from the Hispanic Mother-Daughter Program.
*Workshop absences should be saved for emergencies only.
*Exceptions to this policy may be considered at the discretion of the Program Director. Please Call!
5) The student participant is eligible for one (1) excused absence. Valid reasons for an excused absence
include attending a mandatory school function for a class grade or being sick and providing a doctor’s
note. Refer to the Excused Absence Form available on the HMDP website for further instructions.
REGISTRATION AGREEMENT
Both parent and student participants are required to sign-in at the registration table for each workshop. If a
parent has more than one student participating, the parent must sign-in with each student at the registration
table. If a participant is present but fails to sign-in, the participant will be marked absent.
Be aware that you are ONLY allowed to sign-in for yourself and NOT for any other program participants.
ACKNOWLEDGEMENT OF THE ATTENDANCE POLICY AND REGISTRATION AGREEMENT:
We, the participants of the Hispanic Mother-Daughter Program, fully understand and accept this Attendance
Contract. We understand that if either one of us has more than (1) unexcused absences from the workshops,
we will be removed from the program. We also take full responsibility for signing-in at every workshop, or we
will be marked absent.

CONTRATO DE ASISTENCIA
REGLAMENTO DE ASISTENCIA
El Programa de Madres e Hijas Hispanas requiere que todas las participantes (madre e hija) cumplan con el
reglamento de asistencia. Si una participante no asiste regularmente, no recibirá el beneficio completo del
programa. La Universidad Estatal de Arizona (ASU) y sus respectivos distritos escolares invierten tiempo y
dinero para mantener el programa y por lo tanto, las participantes deben de compartir en la responsabilidad
para asegurarse de su éxito.
El reglamento es lo siguiente:
1) Si la madre o hija no se presenta a uno de los talleres, se considera como una ausencia.
2) Ningún otro individuo puede tomar el lugar del padre o participante sin permiso anterior.
3) Si las participantes llegan a los talleres después de las 7:00 p.m., serán marcadas ausentes.
4) Si ambas participantes (madre o hija) faltan a más de (1) taller durante el año escolar, las dos serán
despedidas del programa.
*Ausencias a los talleres deberían de ser usadas solamente para emergencias.
*Excepciones a estas reglas pueden ser consideradas a la discreción de la Directora del Programa.
5) La hija es elegible para una (1) ausencia justificada. Razones válidas incluyen si la asistencia a una
actividad escolar es obligatoria porque la calificación será afectada o, si está enferma y puede proveer
una nota del doctor. Favor de referir a las instrucciones del documento Permiso de Ausencia
disponible en la página de internet de HMDP.
ACUERDO DE REGISTRACION
Se requiere que la madre e hija se registren en la mesa de registro al llegar a cada taller. Las madres que
tienen más de una hija participando en el programa, deben de registrarse con cada hija en la mesa de registro.
Si están presentes y no se registran, se les marcará como ausente.
Recuerde que SOLAMENTE puede registrase por sí misma y NO se permite registrar a la otra persona.
RECONOCIMIENTO DEL REGLAMENTO DE ASISTENCIA Y ACUERDO DE REGISTRO:
Como participantes del Programa de Madres e Hijas Hispanas, comprendemos y aceptamos este Contrato de
Asistencia. Comprendemos que podemos ser despedidas del programa si cualquiera de nosotras tiene más de
(1) ausencia no justificada. También tomamos la responsabilidad de registrarnos en cada taller y sabemos
que si no nos registramos, seremos contadas ausentes.
