REGISTRATION DOCUMENTS REQUIRED
Dear Parent/Guardian:
There are State and District requirements regarding the enrollment of your child in
Canutillo Independent School District. Most of these documents are already in your child’s
cumulative file. However, the documents/information listed below need to be
current at all times. Please bring them with you when registering your child.
1. Your child must be a resident within the boundaries of
CANUTILLO ISD (We need PROOF OF RESIDENCY
such as a current utility bill in your name or in the
name of the person you are living with)
2. Your child’s IMMUNIZATION RECORD MUST BE UP
TO DATE (see list of all immunizations that are
required)
3. PARENT/GUARDIAN IDENTIFICATION (The
district shall have record of the person with legal
control who is enrolling the child)

The items that are checked above as pending are what you will need to update and bring
to us before your child’s registration will be complete.
It is very important that we have your child’s registration complete as soon as possible so
as not to hinder his/her enrollment. Therefore, we are requesting that you submit to us
the pending information that is checked above as soon as possible.

REQUISITOS DE INSCRIPCIÓN
PARA ESTUDIANTE DE SEXTO Y NOVENO GRADO
Estimado Padre de Familia o Tutor:
Existen requisitos Estatales y del Distrito para inscribir a su hijo(a) en el Distrito Escolar
de Canutillo. La mayoría de la documentación requerida existe ya en el expediente
académico de su hijo(a). Sin embargo, los siguientes documentos e información
deben estar al corriente en todo momento. Le pedimos que los traiga consigo al
momento de registrar a su hijo(a).
1. Su hijo(a) debe ser residente del Distrito Escolar de
Canutillo (Requerimos COMPROBANTE DE DOMICILIO
como recibo actual de agua, luz o gas a nombre de
usted o a nombre de la persona con quien vive)

2. La CARTILLA DE VACUNACION deberá estar al
corriente (ver la lista de todas las vacunas requeridas)

3. IDENTIFICACION DEL PADRE O TUTOR (el distrito
debe tener registro de la persona con capacidad legal
que solicita la inscripción)

Los puntos que se indican arriba como pendientes son los que usted necesita actualizar y
traer a la escuela para la inscripción definitiva de su hijo(a).
Es importante que llevemos a cabo la inscripción completa de su hijo lo antes posible,
para no obstaculizar el proceso. Por lo tanto solicitamos nos presente la información
pendiente indicada arriba lo más pronto posible.
