REGISTRATION FORM
REGISTRATION POLICY
1. Deadline for registration is Friday, January 25, 2008.
2. To register, please contact your parish coordinator of altar servers, or other designated parish
leader, for the Registration Form.
3. Each altar server must also complete and submit to the coordinator or designated parish leader
the Parental Consent / Emergency Medical Release Form.
4. The parish leader should collect the Parental Consent / Emergency Medical Release Form and
bring them to the event.
5. The parish leader should maintain a list of altar servers registered for the event.
6. Return the completed Registration Forms to your parish coordinator of altar servers or other
designated parish leader, who will submit the Registration Forms to the Diocese of Austin,
Vocation Office, P. O. Box 13327, Austin, TX 78711
7. No unaccompanied minors will be admitted.
8. Recognition certificates will be provided only to those servers who attend the event.

FORMA DE REGISTRO
NORMAS PARA EL REGISTRO:
1. La fecha límite para el registro será el viernes 25 de enero del 2008.
2. Para registrarse, por favor acuda a su coordinador de monaguillos u otro líder parroquial para
solicitar la Forma de Registro.
3. Cada monaguillo debe también llenar y entregar al coordinador u otro líder parroquial la Forma
para el Consentimiento de los Padres de Familia y Permiso para Proporcionar Información
Médica en Casos de Emergencia.
4. El líder parroquial debe juntar las Formas de Consentimiento y llevarlas al evento.
5. El líder parroquial debe mantener una lista de los monaguillos registrados para el evento.
6. Devuelva la Forma de Registro completa al coordinador de monaguillos de su parroquia u otro
líder parroquial designado, quien a la vez la remitirá a la Diócesis de Austin, Oficina de
Vocaciones P.O. Box 13327, Austin, Tx. 78711.
7. No serán admitidos los menores que asistan sin acompañantes.
8. Los certificados de reconocimiento serán proporcionados solamente a aquellos monaguillos que
hayan asistido al evento.
