CLUBS FALL 2015 REGISTRATION IS OPEN UNTIL 9/17/15.
After this PTA deadline, you must contact the club directly for enrollment.

Please note if you are requesting a scholarship, you MUST include partial
payment and the Scholarship Application Form.
Advanced registration is required, and priority placement will be given to those
that register in time in the order received. Hard copies of Forms are available
in the school office.
Please notice that all checks are to be made payable directly to the club provider, not
the PTA. Thank you.
Actors Gymnasium Club Liability Waiver- if registering for Actors
Gymnasium, this waiver completion is required prior to 1st day.
Chess Club will renew each trimester this year, it is no longer a full year
registration.
Taekwon Do Liability Waiver, Required - INFORMACIÓN DE REGISTRO.
The waiver is also within online registration directly to Connelly's Academy.

instructors. Paper forms are being sent home by instructors to those currently
enrolled, and a supply will be provided in the office.
SCHOLARSHIP INFORMATION
In an effort to make these programs financially accessible to as many
students as possible, the Washington PTA offers partial scholarships to
families eligible for free or reduced lunch. If you would like to apply for a
scholarship, please fill out this SCHOLARSHIP APPLICATION FORM and
return it with the club registration form and partial payment.
A separate scholarship application must be completed for each club for which
you are registering. Any registration form that is turned in without partial
payment or Scholarship Application Form will be returned.

En un esfuerzo por hacer que estos programas sean financieramente
accesibles a tantos estudiantes como sea posible, la PTA de Washington
