E Para un cirujano, la mayor satisfacción es cuando salva la vida de su paciente, ¿se siente satisfecho al cumplir los deseos de sus pacientes? Yo tengo una gran satisfacción. La satisfacción del médico estético es proporcional al grado de satisfacción del paciente. Cuanto más satisfecho está el paciente, tanto más está satisfecho el médico con sus resultados. Los tratamientos que se hacen en medicina estética, aunque sean tratamientos estéticos también redundan en la salud de la persona porque esa mejora estética produce una satisfacción, una mayor autoestima, lo cual redunda en la confianza del paciente en sí mismo y en una mejor actitud de esa persona ante cualquier acto de la vida. Por lo tanto, la satisfacción del médico es directamente proporcional a la del paciente. Es la dicha mutua que se siente cuando el paciente obtiene los resultados que buscaba. La satisfacción es siempre cuando uno logra lo que busca. Si uno se marca objetivos muy difíciles de cumplir y no los cumple, eso es decepcionante. Si se marca objetivos y los cumple, aunque sean simplemente, como en nuestro caso, que el paciente logra perder 5 kilos -a lo mejor para un paciente 5 kilos no es nada, pero para otro es muchísimo- es una gran satisfacción. Para un paciente a lo mejor no significa nada, mejorar el contorno de la nariz o quitarle una arruga en entrecejo y para otro es una gran satisfacción. A veces las pequeñas cosas pueden producir tanta satisfacción como las grandes. Pienso que las personas tienen que tener la esencia para poner la actitud de buscar grandeza en las pequeñas cosas. No hay que hacer grandes cambios en la persona. Sobre todo nunca hay que cambiar el carácter ni el aspecto de la persona. Podemos mejorarlo pero nunca cambiarlo. 154 Son Vida Son Vida 155 belleza belleza Un médico estético puede ser el médico de familia que vaya informando, aconsejando y sobre todo previniendo el envejecimiento. Su clínica se especializa en tratamientos de medicina estética y acupuntura, cuál es la diferencia más grande entre cirugía plástica y medicina estética? La medicina estética y cirugía plástica son complementarios pero tratan aspectos totalmente diferentes. Hay intervenciones quirúrgicas que pueden tener una alternativa en la medicina estética, pero hay otros que no tienen alternativa. En este caso tenemos que acudir al cirujano. Cada tratamiento debe hacerlo la persona más competente. La medicina estética hace tratamientos más delicados, menos agresivos, pero que requieren un mantenimiento. Lógicamente con paso del tiempo los cambios avanzan. Si nosotros quitamos una arruga o mejoramos la imagen de una persona durante un tratamiento completo y no volvemos a hacer nada, con el paso de tiempo esa flacidez, esa arruga va a volver. Uno de los conceptos básicos de medicina estética es que para que un tratamiento tenga un resultado positivo (mejorar y no volver a empeorar) requiere un mantenimiento constante a diferencia de la cirugía plástica que es un tratamiento más puntual, naturalmente más agresivo que se realiza en el quirófano con una anestesia, pero por el contrario lo olvidas para 4-5 años. En cambio, en la medicina estética es un tratamiento más constante, mas mantenido requiere la visita en el gabinete 2-3 veces al año para hacer este tipo de mantenimiento. La medicina estética puede considerarse incluso como una medicina de familia. Un médico estético puede ser el médico de familia que vaya informando, aconsejando y sobre todo previniendo el envejecimiento. Es mucho más fácil hacer tratamientos preventivos que paliar o curar lo que se ya ha estropeado. ¿Cuáles son los tratamientos con más demanda actualmente? La mayor demanda se produce en los tratamientos faciales. Ha habido un cambio en los últimos años; anteriormente había mucha más demanda en los tratamientos corporales, en cuanto a celulitis, flacidez, sobrepeso, las estrías y sigue manteniéndose esta demanda, pero hemos notado un aumento en cuanto a los tratamientos faciales, a mejorar el aspecto exterior y la “tarjeta de visita” de la persona. Gozan de mucha popularidad tratamientos tales como revitalización facial, a base de las vitaminas, tratamiento de relajación del rostro a base de pequeñas implantaciones de bótox que lo que intentan es relajar la expresión, nunca paralizar o embolizar. El avance técnico en esos últimos años ha hecho que los tratamientos faciales sean más seguros, con muchos menos efectos segundarios. Por ejemplo, los láseres que había anteriormente, que tenían gran resultado en tratamientos faciales, provocaban una baja laboral y social de 20-25 días, en los cuales prácticamente debido a ese efecto secundario, no se podía salir de casa. Acabo de 25 días el paciente estaba muy bien, con la piel fantástica, pero poca gente aceptaba 30 días de baja social o laboral por una cuestión estética. Actualmente, los láseres que empleamos se llaman fraccionados o pixelados, lo que significa que el láser trata la piel fraccionadamente y eso tiene un detalle muy importante: la parte de la piel no afectada ayuda a la recuperación inmediata de la piel afectada. Por lo cual en 2-3 días la piel está perfecta. Esas técnicas actuales, con menos efectos secundarios, con menos complicaciones y menos dolorosos, han hecho que aumente la demanda de tratamientos faciales y anti envejecimiento o de prevención del envejecimiento. ¿Existen tratamientos que se realizan más en algún momento del año? Es verdad que hay temporadas en las cuales se observan algunos tratamientos más que otros pero vienen determinados por el propio clima. El sol, que es un agente oxidante importante, impide la realización de ciertos tratamientos como los de láser o tratamientos vasculares (para quitar las varices). Sin embargo, como he mencionado antes, con el avance tecnológico esa situación va cambiando. Hoy en día disponemos de algunos láseres para depilación de vello que se puede hacer en pleno verano y con la piel bronceada. Cuando hasta hace 2 años, eso era imposible. También hay algunos tratamientos corporales que son menos frecuentes en invierno al ser más frecuentes en la primavera. Al cara al verano la gente quiere estar mejor corporalmente para poder lucir mejor de buena silueta en bikini o ropa veraniega. Con lo cual, los meses de febrero, marzo, es decir previos a la primavera, es cuando se da más este tipo de tratamientos. Posteriormente al verano con los efectos oxidantes del sol, hacemos más tratamientos de recuperación. La medicina estética es una disciplina que evoluciona en una manera impresionante, ¿cuáles son los tratamientos que ha introducido en su trabajo recientemente y que revolucionan el mercado? Creo que uno de los tratamientos que más disfruto de realizar, son los tratamientos faciales, con láser, la remodelación sin cirugía, el láser pixelado para uniformizar el color y la textura de la piel, con el cual podemos combinar hasta 4-5 tipos diferentes de láser para mejorar todos esos resultados. Estamos ya iniciando tratamientos a base de aumento de pecho y aumento de glúteos sin requerir ¿Puede ser que la medicina estética tenga más de filosofía oriental y la cirugía plástica sea más del mundo occidental? No creo que la diferencia sea tan dramática. Estas dos disciplinas no están tan divididas. La filosofía oriental es un modo de vida, realmente diferente a la medicina del mundo occidental. En el mundo de la filosofía occidental es donde más se ha desarrollado la medicina estética. EE UU o algunos países de América Latina (Brasil, Venezuela) son los más avanzados en esta disciplina, hay más demanda en medicina estética, mucho mayor que en los países orientales. El país donde más se realiza la medicina estética es Brasil y el segundo, España. 156 Son Vida Son Vida 157 belleza Si es cierto que hay mayor número de mujeres en las consultas de medicina estética, pero la estética de hombre va cogiendo mucho auge porque también están saliendo técnicas nuevas especificas para hombres. quirófano. Eso se hace mediante la implantación de una sustancia llamada ácido hialurónico que es una sustancia natural en el cuerpo que se emplea también para hacer rellenos, aumentos o perfilados de labios o para suavizar los surcos de expresión. Eso lleva muchas ventajas porque no requiere anestesia, ni convalecencia. Se hace con anestesia local y el tamaño del pecho está hecho a voluntad de la paciente. Lo que hay que decir que es temporal. Es reabsorbible y al cabo de un cierto tiempo se pone una cierta cantidad más para mantener el aumento. Creo que es una técnica que puede ser realmente revolucionaria. Mallorca, una isla que atrae gente que se preocupa por su bienestar físico, tiene que ser sitio perfecto para la clínica de medicina estética. ¿Cuál es el perfil de sus clientes? El perfil del paciente habitual en nuestras consultas no suele ser un turista que viene a pasar unas vacaciones y de paso se hace un tratamiento. Naturalmente que tenemos gente que se viene de vacaciones y aprovecha esas vacaciones para hacerse algún tratamiento pero deben ser personas conocidas, amigos de algún paciente nuestro que les contó sobre la clínica. Nuestros clientes son residentes de la isla, de todo tipo de condición social. La medicina estética es una disciplina democrática, en cuanto a que se usa en cualquier ámbito social. No es solo para gente de gran poder adquisitivo. Todos los ámbitos de la sociedad pueden someterse a algún tratamiento de medicina estética. Los hay naturalmente muy caros pero hay también los que son asequibles. Por eso la medicina estética, como me gusta decir, “es democrática”. Si es cierto que hay mayor número de mujeres en las consultas de medicina estética, pero la estética de hombre va cogiendo mucho auge porque también están saliendo técnicas nuevas especificas para hombres. El hombre es el elemento de la sociedad que naturalmente responde a las avances de la sociedad. 158 Son Vida ¿Ser bello es sinónimo de ser feliz? En absoluto. Primero, ¿qué es la belleza? ¿Quién decide lo que es bello? Nadie puede definir la belleza. Sin embargo todo el mundo lo sabe reconocer. Lo que es bello para uno, puede ser que no sea bello para otro. Uno va a sentirse feliz por apreciar las cosas pequeñas y una persona a lo mejor no tan bella tiene otras características que le hacen mucho más feliz. La felicidad no la da ni siquiera ni el amor, ni la salud, sino todo junto. Es un conjunto de cosas, es un sentimiento individual. No por ser más bello uno se siente más feliz. RESTAURANTE JAPONÉS Hoy en día nos encontramos con personas adictas a las cirugías plásticas, la mayoría de nosotros tenemos miedo de envejecer, ¿le pasó alguna vez desaconsejar a alguien un tratamiento que le parecía inútil? No una sino muchas veces. Es verdad que en la medicina estética en la vida que uno va mejorando tiene tendencia de pedir más, más y más. Que el médico sepa frenar y parar al tiempo y no dejar que un paciente se someta a un tratamiento que no le va a ser positivo es un elemento importantísimo de nuestro trabajo. El médico tiene que saber parar y del hecho hemos rechazado muchos tratamientos porque no eran necesarios a pesar de la demanda del paciente. El paciente con un labio perfecto, no tiene porque aumentarse más el labio. Un paciente con una figura correcta no tiene porque adelgazar más. La delgadez no es sinónimo de mas belleza y salud. Por lo tanto forma parte del trabajo del médico estético saber frenar al paciente en un momento dado. Dígame una persona del mundo actual que según usted sea bella. Mi mujer y mis hijas... Hay unos cánones de belleza y todos reconocemos una serie de personas bellas. Si hablamos de celuloide, a George Clooney todo el mundo le encuentra bello. Que a Brad Pitt todo el mundo le encuentra bello. Que a Angelina Jolie, todo el mundo la encuentra bella Que a Scarlett Johansson todo el mundo la encuentra bella. Es decir, hay una serie de personajes que a muchísima gente les encuentra bella y tienen una definición estructural de la cara. Esos cuatro personajes que mencioné tienen caras con unas formas triangulares, mandíbula marcada, saliente, labios turgentes, etc. Digamos que esos son los cánones de belleza que todo el mundo acepta como bello. D The biggest satisfaction for a surgeon is when a patient's life is saved. Do you feel satisfied when you grant your patients' wishes? I feel greatly satisfied. The satisfaction of the aesthetic medicine doctor is proportional to the patient's satisfaction. The more satisfied the patient is, the more satisfied the doctor will be with the results. The treatments performed in aesthetic medicine, although aesthetic, do have an effect on a person's health because that aesthetic improvement produces a satisfaction, higher self-esteem, and this in turn affects the patient's confidence in him or herself and their attitude towards general acts of life. That is the mutual feeling that one feels when the patient gets the results they were looking for. Satisfaction is achieving what one is looking for. If difficult-to-achieve objectives are set and not met, you are disappointed. If you set goals and achieve them, even if they are as simple as the patient losing 5 kilos, it's a great satisfaction as 5 kilos for that particular patient may mean a lot. Sometimes the small things can produce as much satisfaction as the big ones. I think that people need to have an essence that allows them to see greatness in the small things. You don't have to make big changes in a person. We should never change the character or the appearance of the person. We can make it better, but never change it. Your clinic specializes in aesthetic medicine and acupuncture treatments. What is the biggest difference between aesthetic medicine and plastic surgery? Aesthetic medicine and plastic surgery are complementary but deal with vastly different aspects. There are surgical procedures that could very well have an alternative in aesthetic medicine, but there are some that don't. Each treatment has to be performed by the most capable person. Aesthetic medicine deals with more delicate, less aggressive treatments, but does require maintenance. Logically, time advances and that brings on signs of ageing. If we remove a wrinkle or improve a person's image during a certain treatment and then we do nothing, that sagging or wrinkle will return with time. One of the basic concepts in aesthetic medicine is that in order for a treatment to have a positive result (get better and not regress) it requires a regular maintenance. This is different to plastic surgery, an on the spot treatment, naturally more aggressive performed in an operating room under anesthesia, and that you can forget about for 4 or 5 years. Aesthetic medicine requires consistency, a visit to the clinic 2 or 3 times a year in order to perform maintenance. An aesthetic doctor needs to be like a family doctor in that he or she must inform, give advice and above all avoid ageing. It's much easier to do preventive treatments than to treat something that has already gone wrong. Would you consider that aesthetic medicine has more of an eastern philosophy and that plastic surgery more of a western approach? I don't think that the difference is so dramatic. These disciplines are not that divided. Eastern philosophy is a way of life, really different from western medicine. Aesthetic medicine has been developed mainly in the western world. The US and other Latin American countries (Brazil, Venezuela) are the most advanced in this discipline; there is a larger demand for aesthetic medicine, much higher than in eastern countries. Brazil is the country where this medicine is most practiced and Spain comes second. 160 Son Vida Son Vida 161 beauty beauty It is true that the majority of patients are female but male aesthetics are growing because new male specific techniques are emerging, like specific facial treatment, wrinkle ironing and body hair removal. Which treatments are more in demand nowadays? The biggest demand is for facial treatments. There has been a change in the last few years; there used to be a much higher demand for body treatments (for cellulites, sagging, weight reduction, stretch marks) and while demand still exists, we have seen an increase in regards to facial treatments, to improve one's exterior image, their business card so to speak. Treatments such as facial revitalization (vitamin based) and facial relaxation (small Botox injections to relax the expression and never to paralyze it) have gained popularity. Technical advances have made facial treatments safer with fewer side effects. For example: previous lasers that had excellent effects for facial treatments required about 20 to 25 days of labor and social leave, during which this side effect would prevent you from leaving your house. At the end of that period, the patient would be great, with great skin, but few people were willing to stop their work or social lives for aesthetic reasons. Nowadays, we use lasers called fractional or pixilated which means that the laser treats the skin fractionally and that has an important detail; the non affected skin helps the affected skin recover faster. In 2 or 3 days the skin is perfect. These current techniques (with fewer side effects, fewer complications and being less painful) have increased demand for facial and anti-ageing/ageing prevention treatments. Are there any treatments that are more common during certain times of the year? It is true that there are some times of the year when we see some treatments more than others but they are mainly determined by the weather. The sun, an important oxidant agent, prevents the use of certain treatments like laser or vascular treatments (to remove stretch marks). On the other hand, as I mentioned earlier, the situation has changed due to technical advances. We now have, at our disposal, some hair removal lasers that can be used during the summer even on tanned skin. This was impossible two years ago. We also find some body treatments that are less frequent in the winter than in the spring. With summer approaching, patients want to have a better body to be able to display their bikini silhouette or other summer clothes. The months prior to the spring, February and March, are when we see a lot of these body treatments. After the summer and the oxidant effects of the sun, we do more recovery treatments. Aesthetic medicine is a discipline that evolves at an impressive rate. What treatments have 162 Son Vida you recently included in your work and that have revolutionized the market? I think that the treatments I enjoy giving the most are the facial ones, with laser, rinomodeling (nose) without surgery, pixilated laser for uniform coloring and skin texture, with which we can use 4 or 5 laser types to improve all the results. We are now starting some breast and buttocks treatments without surgery. This is done by implanting a substance called hialuric acid (Macrolane) which is a natural substance in the body and it is also used to fill, augment or profile lips or to smooth out expression lines. This implies many advantages as you do not need anesthesia or a hospital stay. It is performed with local anesthesia and the breast size is determined by the patient. It is, however, temporary. It is absorbed by the body and after a while another quantity is implanted to maintain the size. This can be a truly revolutionizing technique. Mallorca, an island that attracts people who are worried about their physical health, must be a perfect place for aesthetic medicine clinics. What is your patient profile? The usual patient profile for our offices is not the tourist that comes here for vacation and decides to have a treatment. We do have some of those but they are usually friends referred by our patients. Our clients are residents of all social classes. Aesthetic medicine is a democratic discipline in that it is used in any social environment. It's not only for people with an elevated purchasing power. All social classes can be treated with aesthetic medicine. Some of the treatments are very expensive but some are rather cheap as well. It is true that the majority of patients are female but male aesthetics are growing because new male specific techniques are emerging, like specific facial treatment, wrinkle ironing and body hair removal. Women, and naturally men as well, are responding to how society advances. Is beauty synonymous with happiness? Not at all. First, what is beauty? Who decides what is beautiful? It is hard to define beauty, however everyone can recognize it. What is beautiful to one may not be beautiful to another. One can feel happiness appreciating the little things and that person, maybe not that beautiful, has the capability to enjoy that maybe the beautiful person doesn't. Happiness is not reached through beauty, nor love, nor health or money. It is a combination of these things, an individual feeling. Being more beautiful doesn't mean one will be happier. However, a happy person will find themselves to be beautiful. Nowadays we can find people addicted to plastic surgery, most of us are afraid to get old. Have you ever advised against a treatment because you though it was useless? Not once, but many times. It is true with aesthetic medicine that when one gets better, there is a tendency to ask for more and more. The ability of a doctor to be able to brake and stop and not let a patient undergo a treatment that will not be positive is a very important element in our line of work. The doctor must know when to stop and we have refused to perform many treatments requested by our patients. The patient with a perfect lip doesn't need to increase its size. A patient with a good body doesn't need to loose more weight. Being thin is not synonymous with beauty and health. Being able to stop the patient is part of an aesthetic doctor's job. Tell me about one person that, according to you, is beautiful. My wife and daughters… There are some beauty standards that we all recognize. If we talk about the movies, we can see that everyone finds George Clooney to be beautiful, the same with Brad Pitt, Angelina Jolie and Scarlett Johansson. There is a series of characters that everyone finds beautiful because they have a well defined facial structure. Those four people I mentioned all have triangular faces, defined jaws, thick lips, etc. Let's say those are the beauty standards that everyone recognizes. However, I cannot find George Clooney or Angelina Jolie to be the most beautiful people on earth because there is so much more to beauty than the physical appearance. ■
