Datos recientes indican que la EEC es un trastorno clonal, caracterizado por mutaciones BRAFV600E recurrentes en > 50% de los pacientes, donde la inflamación crónica no controlada es un mediador importante en la patogenia de la enfermedad. (Blood. 2014;124(4):483-492) Introducción La enfermedad de Erdheim-Chester (EEC) es una rara histiocitosis de tipo no Langerhans descrita por Jakob Erdheim y William Chester en 19301. Aunque se han descrito 500 a 550 casos, el número ha aumentado espectacularmente en los últimos 10 años debido a un mejor diagnóstico de la enfermedad. Históricamente, la EEC se ha considerado como un trastorno histiocítico de agresividad variable y de origen poco claro con mala respuesta al tratamiento. No obstante, la reciente identificación de la naturaleza clonal del trastorno, con al menos una mutación somática oncógena recurrente de importancia terapéutica, ha reformulado nuestro conocimiento de la patogenia y el tratamiento clínico de la enfermedad2. En la actualidad no hay directrices aceptadas de forma unánime para el diagnóstico y el tratamiento de la EEC y los pacientes acuden a diversos especialistas médicos. Además de las características histológicas, en la EEC está casi siempre presente el hallazgo radiológico de osteoesclerosis diafisaria y metafisaria simétrica en las piernas (Figura 1A-D). La mejor visualización se obtiene mediante captación de radiotrazadores en las extremidades distales de los fémures y en las porciones proximal y distal de las tibias en la gammagrafía ósea y, con menos sensibilidad, mediante la tomografía de emisión de positrones (PET). Las lesiones óseas se pueden visualizar mediante tomografía computarizada (TC) o resonancia magnética (RM), pero a menudo pasan desapercibidas en las radiografías simples22,23. En el 4% aproximadamente de los pacientes faltan los signos radiológicos de osteoesclerosis de los fémures15. En estos casos, el diagnóstico de EEC se basa en la histopatología y en la afectación de otros órganos clásicos. En segundo lugar, la infiltración densa de grasa perinéfrica, descrita como “riñón peludo” por su aspecto en la TC, es un signo radiológico muy prevalente (68% de los casos) e icónico (Figura 2A)22. Pero incluso en circunstancias de características clínicas y de imagen muy sugerentes, es necesaria la biopsia para confirmar el diagnóstico y establecer el estado de mutación de BRAF (grado 2C). Características clínicas y radiográficas. En la Figura 3 se presentan las frecuencias estimadas de los hallazgos clínicos y radiológicos en los pacientes con EEC según la experiencia más extensa en un solo centro hasta la fecha15. Es frecuente el diagnóstico retrasado o erróneo, y desde el comienzo de los síntomas hasta el diagnóstico pueden transcurrir meses o incluso decenios. Como se ha mencionado, la EEC comparte características clínicas con la HCL (Tabla 2) pero guarda poca semejanza clínica con otras histiocitosis no HCL, con la excepción de la histiocitosis sinusal con adenopatías masivas (enfermedad de Rosai-Dorfman)24,25. Aunque en la EEC se ha descrito infiltración de casi todos los sistemas orgánicos26‑31, los tejidos afectados con más frecuencia son los siguientes: esqueleto, retroperitoneo y órbita, además de los sistemas cardiovascular, pulmonar, neurológico y endocrino (Figura 3). A pesar de que los huesos largos están casi siempre afectados en la EEC, solo el 50% de los pacientes refieren dolor óseo32. La distribución de las lesiones escleróticas en la EEC difiere de la observada en la HCL en que esta última afecta habitualmente a cráneo, huesos faciales, porción proximal de extremidades, pelvis y escápula y no a la porción distal de las extremidades como en la EEC33. Manifestaciones cardiovasculares. La afectación cardiovascular es frecuente, pero a menudo asintomática y detectada casualmente mediante TC o RM34,35. La anomalía más frecuente es el revestimiento circular de la aorta torácica y abdominal y de sus ramas por tejido blando, visible en la TC, la denominada “aorta tapizada”, presente hasta en dos tercios de los pacientes (Figura 2A)15,36. Hay infiltración mural pseudotumoral de la aurícula derecha hasta en un tercio de los pacientes, que se visualiza claramente en la RM como una masa (Figura 2B), que rara vez causa disfunción valvular y anomalías de la conducción34,35. Se ha observado, aunque no se ha publicado, afectación de las venas intrahepáticas causante de hipertensión portal, así como de los vasos mesentéricos con la consiguiente isquemia. Manifestaciones pulmonares. Puede haber afectación pulmonar radiológica hasta en la mitad de los casos, en el parénquima pulmonar o la pleura (Figura 2C)15,43. Las radiografías simples son habitualmente normales, pero la TC de alta resolución puede mostrar engrosamiento de los tabiques interlobulares, opacidades en vidrio esmerilado u opacidades centrolobulillares22. Intraaxialmente, la EEC se manifiesta con más frecuencia en los núcleos dentados del cerebelo o en la protuberancia, y causa síntomas cerebelosos progresivos como ataxia y disartria, así como síntomas del tronco encefálico (Figura 2E)22,25,46. Estas anomalías tienden a realzarse con gadolinio y pueden confundirse con tumores primarios o metastásicos, enfermedad desmielinizante, procesos inflamatorios o leucodistrofias28,29. Se han descrito lesiones intracraneales similares en el XGJ47. Las lesiones infiltrantes del SNC en la HCL tienen una distribución y aspecto radiológico similares a las lesiones en la EEC, con la excepción de las lesiones meníngeas, que son más expansibles en la EEC, y de la médula espinal, que está preservada en la HCL. Un proceso atrófico degenerativo de la fosa posterior es una manifestación poco frecuente pero bien descrita de la HCL48; los fenómenos neurodegenerativos en la EEC siguen sin estar definidos. Se produce infiltración unilateral o bilateral de las órbitas en ~ 25% de los pacientes, que puede presentarse como exoftalmos, dolor retroorbitario, parálisis oculomotoras o ceguera15,49. Es necesario hacer el diagnóstico diferencial de estas lesiones pseudotumorales con procesos como enfermedad de Graves, enfermedades granulomatosas, linfomas y arteritis de células gigantes. La diabetes insípida es una característica que comparten la EEC y la HCL y que aparece en ~ 25% de los pacientes con EEC, aunque se han descrito varias endocrinopatías, como hiperprolactinemia, insuficiencia de gonadotropinas e hipotestosteronismo15. Radiológicamente, la hipófisis, el tallo y el hipotálamo pueden ser normales; otras veces es posible apreciar en la RM aumento de tamaño y realce anormal, con o sin endocrinopatía acompañante (Figura 2F)50. Manifestaciones retroperitoneales. Es frecuente una infiltración de los tejidos perinéfricos que da origen al “riñón peludo”, al igual que la hidronefrosis y la estenosis ureteral (Figura 2A). Además, ~ 30% de los pacientes con ECC presentan características de imagen indicativas de fibrosis retroperitoneal; sin embargo, a diferencia de la fibrosis retroperitonal idiopática, los uréteres pélvicos y las venas cavas inferiores están habitualmente preservados15. En caso de que la hidronefrosis se acompañe de insuficiencia renal puede ser necesaria la colocación de stents ureterales. Manifestaciones cutáneas. La lesión cutánea más frecuente en la EEC es el xantelasma, que se presenta como placas de color amarillo en los párpados en un tercio aproximadamente de los pacientes15. Otras localizaciones son cara, cuello, axila, tronco e ingles, donde pueden aparecer pápulas de color amarillo o rojo-marrón que surgen en las placas. Es imposible distinguir entre EEC y XGJ del adulto basándose exclusivamente en las lesiones cutáneas aunque, a diferencia de la EEC, la XGJ es con menos frecuencia una enfermedad multisistémica25. Categorías de enfermedad propuestas. Se ha comunicado que los pacientes con enfermedad del SNC tienen una evolución adversa con el tratamiento basado en interferón en comparación con los pacientes que presentan otras localizaciones de la enfermedad, lo que respalda la idea de que Tabla 3. Una localización de la enfermedad puede dominar la presentación clínica y requerir un tratamiento focalizado además del tratamiento de la EEC subyacente. Rara vez, algunos pacientes tienen una enfermedad poco activa y asintomática que justifica un control expectante en lugar de un tratamiento activo. En consecuencia, proponemos clasificar la EEC en asintomática y sintomática (Tabla 3). La EEC sintomática se puede clasificar a su vez como de predominio en el SNC, predominio cardíaco, predominio vascular, predominio endocrino, predominio retroperitoneal, predominio pulmonar y multisistémica. La afectación intensa de prácticamente cualquier órgano y sistema constituye una enfermedad de “alto riesgo”; por tanto, el órgano más afectado fisiopatológicamente es el que mejor caracteriza el fenotipo clínico de cada paciente. Creemos que esta clasificación potencia la descripción de esta enfermedad multiforme, facilita el tratamiento adecuado y es útil para el desarrollo de nuevos tratamientos. Evaluación basal y valoración de la mutación BRAF V600E Evaluación basal. En la Tabla 4 se presenta la evaluación inicial recomendada de los pacientes. En todos los pacientes se recomienda TC de tórax, abdomen y pelvis, PET con (18F)-fluorodesoxiglucosa (FDG) de todo el organismo, incluido el cerebro y la porción distal de las extremidades, RM cerebral con gadolinio y RM cardíaca (grado C2). Pueden ser convenientes otros estudios para evaluar hallazgos clínicos o analíticos o para definir anomalías de imagen de forma detallada (grado C1). Aunque la gammagrafía ósea con 99mTc facilita el diagnóstico de EEC al demostrar las lesiones icónicas, la sensibilidad de la PET en la afectación extraósea ha convertido a la PET-FDG en el estudio de medicina nuclear de elección para la evaluación de la carga global de la EEC51‑53. Se realizan estudios analíticos para evaluar la existencia de insuficiencia renal, citopenias, marcadores de inflamación y signos de endocrinopatía (grado C1). El objetivo de esta evaluación basal integral es caracterizar la carga de la enfermedad y detectar anomalías que pueden surgir clínicamente en el futuro. Obtención de tejido lesional. La selección del lugar de la biopsia y la confirmación histopatológica de ECC son complicadas. La biopsia por raspado de lesiones cutáneas como el xantelasma es el procedimiento menos invasivo y a menudo proporciona tejido diagnóstico adecuado. Con independencia del sitio de biopsia, se intentará obtener varias muestras, ya que el rendimiento de una única biopsia puede ser bajo, dado que las alteraciones varían de un campo a otro. Además, para la interpretación histopatológica es preciso descalcificar las muestras óseas, lo que deja el material inservible para análisis genéticos; por tanto, es fundamental obtener un gran número de muestras y conservar material fresco sin descalcificación para la extracción de ADN. Los datos histopatológicos clásicos de la EEC ya se han comentado; sin embargo, es preciso señalar que el tejido lesional a menudo no muestra el clásico infiltrado histiocítico espumoso. Con frecuencia se detectan una serie de hallazgos, como inflamación inespecífica con fibrosis entremezclada o incluso fibrosis aislada con escasos histiocitos (Figura 4). Estudio de la mutación BRAF V600E. Dadas las posibles consecuencias terapéuticas de la mutación BRAFV600E, es importante detectarla con exactitud (grado C2). El contenido sumamente variable de histiocitos presente en muchas muestras de biopsia influye en los resultados del estudio de la mutación BRAF11. Además, la frecuencia de las biopsias óseas en la EEC supone un problema adicional para la identificación de las mutaciones BRAF. Por estas razones, es muy aconsejable confirmar la negatividad de la prueba de BRAF V600E en la EEC utilizando > 1 modalidad de genotipificación o la genotipificación de > 1 localización anatómica (en particular cuando se encuentra que las lesiones óseas son de tipo BRAF natural) (grado C1). Hay numerosas pruebas moleculares para determinar la existencia del genotipo BRAF (revisadas recientemente)54. La recomendación de consenso es favorable a PEG‑IFN‑α, y la dosis viene determinada por la gravedad y el predominio del órgano en la enfermedad. Tratamiento dirigido con anticitocinas (anakinra, infliximab, tocilizumab). Como se considera que IFN‑α ejerce un efecto beneficioso al suprimir el efecto de IL‑1, se ha intentado el tratamiento con anakinra, un antagonista recombinante de IL‑1R, que resultado eficaz en un pequeño número de pacientes con resultados publicados y en muchas más experiencias no publicadas. Se ha observado reducción de la carga de la enfermedad, así como de citocinas proinflamatorias, en 5 casos clínicos de tratamiento con anakinra en dosis de 1 a 2 mg/kg al día64‑67. El tratamiento fue bien tolerado y particularmente eficaz para el dolor óseo y los síntomas generales. La cardiopatía se trató con éxito en un paciente67, pero anakinra no ha resultado útil en el tratamiento del SNC en la EEC. Entre los efectos secundarios habituales figuran reacción en el lugar de la inyección, cefalea, artralgias y nasofaringitis. Actualmente hay menos datos científicos que respalden el uso de anakinra como tratamiento de primera línea en lugar de IFN‑α, si bien es un tratamiento razonable en muchos pacientes, especialmente aquellos sin afectación del SNC y con predominio de síntomas óseos o generales (grado D1). Se han investigado otros tratamientos anticitocinas en la EEC, entre ellos infliximab y tocilizumab. Cuatro pacientes con cardiopatía resistente a IFN‑α experimentaron una mejoría clínica y una reducción de las concentraciones de citocinas circulantes al recibir infliximab, un anticuerpo anti-factor de necrosis tumoral. Aunque es necesario estudiarlo más a fondo, puede considerarse un tratamiento de segunda línea (grado D1)68. En la actualidad se está realizando un ensayo clínico en fase II de tocilizumab, un anticuerpo monoclonal humanizado contra el receptor de IL‑6 (NCT01727206). Corticosteroides, quimioterapias citotóxicas, radioterapia e intervención quirúrgica. Antes del descubrimiento de IFN‑α como tratamiento para la EEC se describieron muchas pautas terapéuticas con quimioterapias citotóxicas en pequeñas series, entre ellos alcaloides de la vinca, antraciclinas, ciclofosfamida y quimioterapia en dosis altas con autotrasplante de células progenitoras69‑76. Los corticosteroides pueden reducir el edema de forma inmediata, por ejemplo, en el exoftalmos grave, pero no están considerados como un tratamiento eficaz (grado D2). El análogo de la purina cladribina se ha utilizado en el tratamiento de la HCL multisistémica y en la EEC tanto de reciente diagnóstico como resistente, pero los informes publicados sobre su eficacia son escasos3,77,78. Debe considerarse su uso en el contexto de tratamiento de segunda línea (grado D1). Actualmente se está llevando a cabo un ensayo clínico de tratamiento combinado con sirolimús y prednisona (ACTRN12613001321730). Se ha descrito el uso de radioterapia en la EEC, aunque con una respuesta nula o, como máximo, una respuesta paliativa a corto plazo, con progresión de 490 DIAMOND y cols. la enfermedad en unos meses79,80. La utilidad de la reducción quirúrgica en la EEC se limita a las lesiones orbitarias graves o a lesiones intracraneales resecables quirúrgicamente. Inhibidores de la serina/treonina cinasa: vemurafenib e imatinib. Hasta la fecha se ha descrito el tratamiento de 3 pacientes de EEC con vemurafenib, un inhibidor del BRAF portador de la mutación V600E, con una mejoría clínica y radiológica espectacular y sin precedentes81. La experiencia clínica con vemurafenib en la EEC está aumentando rápidamente, tanto en estudios clínicos prospectivos (NCT01524978 y un estudio en el Hôpital Pitié-Salpêtrière, París, Francia) como en administración extraoficial para indicaciones no autorizadas. Las respuestas publicadas a vemurafenib en la EEC con mutación BRAF indican que debe utilizarse el tratamiento con un inhibidor de BRAF en el contexto de un ensayo clínico en todos los pacientes con EEC que requieran tratamiento y que alberguen la mutación BRAFV600E (grado C2). En la actualidad no hay suficientes datos clínicos para excluir a todos los pacientes sin BRAFV600E del tratamiento con los inhibidores del RAF existentes en la actualidad, pero todavía no se ha intentado. Los efectos tóxicos frecuentes consisten en cansancio, artralgias, cefalea y múltiples complicaciones cutáneas, entre ellas la aparición de carcinomas espinocelulares. Dado que vemurafenib no está aprobado actualmente por la FDA para las enfermedades histiocíticas, se aconseja su inclusión en estudios clínicos en lugar de su uso extraoficial. Cuestiones fundamentales, como la duración óptima del tratamiento, las secuelas de la suspensión del fármaco y los posibles efectos a largo plazo de vemurafenib, se desconocen en el momento actual y son motivos importantes para que el fármaco se administre en el contexto de un ensayo clínico. El mesilato de imatinib se ha utilizado con éxito en casos clínicos de otros trastornos histiocíticos82,83. Aunque ningún trastorno histiocítico tiene mutaciones conocidas en KIT, ABL o PDGFR, algunas lesiones histiocíticas en la EEC y enfermedades relacionadas parecen tener una gran expresión de PDGFR-β84. Los resultados en 7 pacientes tratados con imatinib han sido variables, pero es una estrategia terapéutica razonable cuando fracasan los tratamientos de primera línea (grado C0)84,85. EEC pediátrica. Dado que la EEC es extremadamente poco frecuente en los niños, los datos relativos a su tratamiento se limitan a casos clínicos. Se han descrito buenos resultados con tratamientos a base de IFN en casos aislados y con anakinra en un paciente66,86,87. En dos ocasiones se han obtenido buenos resultados con corticosteroides en monoterapia88,89. Es de señalar que la aplicación directa de los tratamientos del protocolo de HCL a niños con EEC tuvo éxito en 2 casos66,90. Vigilancia de la enfermedad, duración del tratamiento y pronóstico Debe realizarse una PET-FDG cada 3 a 6 meses en todos los pacientes después del inicio del tratamiento (grado C0), y el intervalo entre las exploraciones puede aumentarse una vez que se haya estabilizado la enfermedad91. El estudio de imagen específico de los órganos afectados debe realizarse cada 3 meses inicialmente después de comenzar el tratamiento, cada 6 meses una vez estabilizada la enfermedad, o antes si así lo indica alguna modificación del estado clínico o de las cifras analíticas, como las de función renal (grado C2). No hay biomarcadores séricos específicos de la enfermedad; La proteína C-reactiva está elevada en el 80% de los casos en el momento del diagnóstico, y el seguimiento de su concentración puede ser útil para controlar el tratamiento (Grado D1)15. En la actualidad se recomienda continuar el tratamiento de forma indefinida si es tolerado; no obstante, puede ser razonable, valorando caso por caso, tratar de suspender el tratamiento en los pacientes con una carga de enfermedad mínima o estable durante un período prolongado (grado D2). El tratamiento indefinido mediante inhibición del BRAF es potencialmente poco seguro, dato el riesgo de acelerar lesiones neoplásicas premalignas mediadas por RAS92, aunque hasta la fecha no se ha definido la evolución del tratamiento con vemurafenib en la EEC. El pronóstico publicado de la EEC es desfavorable, con una supervivencia del 43% de los pacientes después de un seguimiento promedio de 32 meses93. En informes recientes en los que se describe la supervivencia de pacientes tratados de modo uniforme con interferón se indica una supervivencia global a los 5 años del 68% en esta enfermedad45. La experiencia BLOOD, 24 DE JULIO DE 2014 • VOLUMEN 124, NÚMERO 4 informal de los autores es que la supervivencia a largo plazo es en la actualidad incluso más prometedora, pero esta impresión no se ve reflejada en la literatura científica reciente. Conclusiones La EEC es un raro trastorno multisistémico que requiere una colaboración multidisciplinaria para su diagnóstico y tratamiento. Estudios recientes han permitido avanzar en la redefinición de la patogenia de este trastorno y en el establecimiento de tratamientos dirigidos sobre bases moleculares e inmunológicas. Para establecer el diagnóstico es necesaria una biopsia que demuestre las características histopatológicas típicas además de las características clinicorradiológicas, casi siempre afectación esclerosante de los huesos largos. Además del diagnóstico se requiere una serie de estudios analíticos y radiológicos para determinar la extensión basal de la enfermedad. Asimismo, en todos los pacientes con EEC es fundamental un análisis de mutaciones que demuestre el estado mutacional de BRAF y RAS, incluso si ello requiere una biopsia adicional, ya que tiene consecuencias para el tratamiento mediante inhibición del BRAF. Se recomienda tratamiento en el momento del diagnóstico en todos los pacientes, excepto en aquellos con enfermedad mínimamente sintomática. Se recomienda encarecidamente el envío a un centro médico docente con experiencia en el tratamiento de la EEC, dada la escasa experiencia clínica con este trastorno.
