Dear Resident,
Your family has been chosen to take part in a phone survey. The survey is being conducted by
the Centers for Disease Control and Prevention (CDC). The results of this survey will give public
health experts information about health issues in the United States.
In the next two weeks, a person from a group called Clearwater Research Inc. will call you. This
group is working with CDC to conduct the survey. When they call, they will ask how many
people live in your house. After that, they will choose a person in your house to answer some
questions. The questions will be about the foods you eat and health issues. If they choose
someone between 12 and 17 years old, a parent or guardian will need to give permission for the
young person to answer the questions. If you are called at a bad time, please let the person
calling know and they will set another time to call you.
The survey will take about 20 minutes. You may choose not to take part in the survey or to
answer only some questions. The survey is confidential which means you or your family will not
be identified in any of the results.

Estimado Residente,
Su familia ha sido escogida para participar en una encuesta telefónica. Esta encuesta es
conducida por los Centros de Control y Prevención de Enfermedades. Los resultados de esta
encuesta van a dar información a expertos de salud sobre los asuntos de salud en los Estados
Unidos.
En las próximas dos semanas, una persona del grupo Clearwater Research, Inc. le llamará. Este
grupo esta trabajando con los CDC para conducir la encuesta. Cuando llamen, le van a preguntar
cuantas personas viven en su hogar. Después, van a escoger a una persona de su hogar para
contestar unas preguntas. Las preguntas son sobre las comidas y asuntos de salud. Si escogen a
alguien entre 12 a 17 años de edad, un padre o guardián tendrá que dar permiso para que la el
menor conteste las preguntas. Si le llaman en una ocasión inoportuna, por favor avísele a la
persona llamando para hacer una cita en otra ocasión.
La encuesta tomará alrededor de 20 minutos. Usted puede decidir si no desea participar en la
encuesta o si desea responder solamente a algunas preguntas. La encuesta es confidencial lo cual
significa que ni usted ni su familia serán identificados en los resultados.
