If you have Medicaid Managed Care,
please read the information below:
HSS participates with Affinity Health Plan, United Healthcare Community Plan and HIP*.
For the most up-to-date information regarding the plans we participate with, please call us or

For more information on what services are covered by the plans, please call Affinity Health
Plan at (866)247-5678, United Healthcare Community Plan at (800)493-4647 or HIP* at
(888) 224-5286.
If you are a member of another Medicaid Managed Care Plan, you will need to get
an out-of-network authorization from your plan. Please have your Primary Care Physician
(PCP) request the authorization from your plan before your scheduled appointment at HSS.
An Out-of-Network Authorization for HSS must include the following:
a. An authorization number from your Managed Care Plan
• X-Rays and Lab Work
• Number of visits authorized
• Name of Clinic authorized
• Name of your PCP and phone number
b. The name of the facility - Hospital for Special Surgery
c. Tax ID # 131-62-4135 or NPI # 159-87-03019
Please contact the VOICES Program at (212)774-2011 once you obtain an authorization or if
you need additional information.
*Please note that HIP requires an in-network authorization for all visits.
