earn your reward

Make an appointment now with your primary care
physician (PCP). Your PCP can write you a prescription
to go to Quest Diagnostics to get your lab tests.
Your PCP also can refer you to your eye doctor for a
diabetic eye exam.

numbers helps you track the progress you’re making
toward a healthier you.
Staying active and eating right helps keep your
blood sugar, blood pressure and cholesterol under
control. It helps you manage your diabetes.

loaded onto the card expire 90 days after they have been awarded.
