Test purpose Seraclone® Anti-s (MNS4) is used to determine the red cell antigen s. Test principle The test principle is a hemagglutination test to determine the respective red cell antigens. The blood type s is defined by presence or absence of the s antigen on red cells. The antigen s reacts specifically with the corresponding antibody in Seraclone® Anti-s (MNS4). A positive reaction is visible by agglutination. The reagent can be used in tube testing. Reagent As reactive component Seraclone® Anti-s (MNS4) contains a human monoclonal antibody of the immunoglobulin class IgG. It derives from cell culture supernatant and demonstrates the consistent specificity and reproducibility characteristic for monoclonal antibodies. Seraclone® Anti-s (MNS4) clone P3YAN3 Performance characteristics and limitations of the method In case of negative results in an indirect antiglobulin-test, a repetition with antibody coated reagent cells should be carried out. Failure of this control can occur due to either errors in the test procedure or a decrease of the reagent’s reactivity. In case of questionable results of unknown origin our Bio-Rad Service will assist you (phone: +49-6103-3130-611). Shelf life After opening the reagent the product can be stored until its expiry date under proper storage conditions (2...8°C). The expiry date is printed on the label. Do not use damaged vials. Warning and precautions Turbidity or other visible changes may indicate a bacterial contamination. In this case the reagent must be discarded. The cause for this change must be examined by the manufacturer. All manual techniques are to be performed according to the manufacturer’s instructions. If not, it is the sole responsibility of the user. Used tests must be discarded as hazardous material. Waste management according to national guidelines. Because of biotechnological manufacturing of human monoclonal antibodies a contamination with infectious agents can be considered impossible. Nevertheless, all test reagents of biological origin must be regarded as potential transmitters of hepatitis, HIV or other infectious agents. Appropriate safety precautions are recommended. Internal quality controls according to national guidelines, e.g. (1), are recommended at regular intervals. As a minimum a positive and a negative control should be used. Fresh, nonhemolytic samples (clotted blood, EDTA, or citrate blood) should be used. If needed prepare a red cell suspension from a centrifuged blood sample (e.g. 2 min. at 800 1000 x g) in isotonic saline solution as suspension medium. We recommend to wash the red cells to be tested prior to preparing the cell suspension at least 2 times or until the supernatant is clear. Strongly lipemic, icteric or microbially contaminated samples may lead to false results. Test procedure Tube test 1. Prepare a 3 – 5 % suspension of the red cells in isotonic saline. 2. In an appropriately marked tube mix 1 drop reagent and 1 drop of red cell suspension. 3. Incubate for 30 minutes at 37°C. 4. Wash red cells three times with isotonic saline solution. Decant supernatant completely. 5. Add 2 drops of anti-human-globulin to the packed red cells and mix. 6. Centrifugate for 20 seconds at 800 - 1000 x g. 7. Gently dislodge cell button and observe for agglutination. Tube test, LISS method 1. Suspend red cells to be tested 3–5 % in isotonic saline solution 2. In a properly marked tube mix 1 drop reagent, 1 drop red cell suspension and 2 drops MLB 2. 3. Incubate for 10 minutes at 37°C . 4. Wash red cells 3 times in isotonic saline solution. Completely decant supernatant. 5. Add 2 drops Anti-Human-Globulin and mix. 6. Centrifuge for 20 seconds at 800 - 1000 x g. 7. Gently dislodge the cell button and observe for agglutination. A positive and a negative control should be performed in parallel (1). Interpretation of results Evaluation of the reaction (2): Reaction strength 4+ 3+ 2+ 1+ +/- strength is carried out according to the Technical Manual Agglutination One single agglutinate. No free red cells. Strong reaction. Some large agglutinates. Medium-size agglutinates, no free red cells. Numerous small agglutinates against a background of free red cells. Just a few macroscopically detectable agglutinates within red cell suspension. Numerous microscopically detectable agglutinates. Homogenous red cell suspension without detectable agglutinates. Verwendungszweck Seraclone® Anti-s (MNS4) wird zum Nachweis des Blutgruppenmerkmals s eingesetzt. Testprinzip Das Testprinzip ist ein Hämagglutinationstest zum Nachweis des entsprechenden Erythrozyten-Antigens. Die Blutgruppe s wird durch das Vorhandensein oder Fehlen des Merkmals s auf den Erythrozyten definiert. Das Antigen s reagiert spezifisch mit dem korrespondierenden Antikörper in Seraclone® Anti-s (MNS4). Eine positive Reaktion wird durch eine Agglutination sichtbar. Das Reagenz kann im Röhrchentest verwendet werden. Reagenz Seraclone® Anti-s (MNS4) enthält als reaktiven Bestandteil monoklonale Antikörper der Immunglobulinklasse IgG humanen Ursprungs. Sie werden aus Zellkulturüberstand gewonnen und weisen für monoklonale Antikörper charakteristische konstante Spezifität und Reproduzierbarkeit auf. Seraclone® Anti-s (MNS4) Klon P3YAN3 Zusätzlich benötigte Reagenzien und Materialen • Pipetten (Tropfenvolumen 40-50μl) • Aqua dest. • Isotone Kochsalzlösung • Anti-Human-Globulin (z. B. Anti-Human-Globulin Color [REF] 804115 und 804130) • MLB 2 ([REF], 805 205, 805 200) • IgG-sensibilisierte Erythrozyten (z. B. Coombscell-E [REF] 816 030) • Glasröhrchen • Laborzentrifuge Probenmaterial Blutproben werden nach allgemeingültigen Entnahmeverfahren gewonnen. Es sollten frische, nicht hämolytische Proben (Nativ-, EDTA- oder Citratblut) verwendet werden. Wird für die Durchführung der Methode eine Erythrozytensuspension benötigt, wird diese aus den Erythrozyten der abzentrifugierten Blutprobe (z.B. 2 Min. bei 800 - 1000 x g) mit isotoner Kochsalzlösung als Suspensionsmedium erstellt. Wir empfehlen, die zu untersuchenden Erythrozyten vor Herstellung der Zellsuspension mindestens 2 mal zu waschen; ggf. so lange, bis der Überstand klar ist. Stark lipämische, ikterische oder mikrobiell kontaminierte Proben können zu unzuverlässigen Testergebnissen führen. | 186988/03 – 08/2012 Testdurchführung Röhrchentest 1. Die Erythrozyten 3 mal mit isotoner Kochsalzlösung waschen. Den Überstand vollständig dekantieren. 5. Dem Sediment 2 Tropfen Anti-Human-Globulin zufügen und mischen. 6. 20 Sekunden bei 800 - 1000 x g zentrifugieren. 7. Unter leichtem Aufschütteln auf Agglutination prüfen. Eine positive und eine negative Kontrolle sollten bei allen Untersuchungen mitgeführt werden (1). Interpretation der Ergebnisse Die Bewertung der Reaktionsstärken erfolgt analog den Vorgaben des Technical Manual (2): Reaktionsstärke Agglutinationsbild 4+ Ein einziges Agglutinat. Keine freien Erythrozyten. 3+ Starke Reaktion. Einige große Agglutinate. 2+ Mittelgroße Agglutinate, keine freien Erythrozyten. 1+ Viele kleine Agglutinate innerhalb eines Hintergrunds freier Erythrozyten. +/Wenige makroskopisch erkennbare Agglutinate innerhalb der Erythrozytensuspension. Zahlreiche mikroskopisch erkennbare Agglutinate. Homogene Erythrozytensuspension ohne erkennbare Agglutinate. Leistungsmerkmale und Grenzen der Methode Negative Ergebnisse im indirekten Antihumanglobulin-Test sollten mit antikörperbeladenen Testerythrozyten überprüft werden. Ein negatives Ergebnis dieser Kontrolle lässt auf einen Fehler in der Testdurchführung oder auf einen Reaktivitätsverlust des Reagenzes schließen. Bei zweifelhaften Ergebnissen unklarer Ursache steht der Bio-Rad-Service (Tel. 06103-3130-611) für Rückfragen zur Verfügung. Haltbarkeit Nach Öffnung des Reagenzes ist das Produkt bei sachgemäßer Lagerung (2...8°C) bis zum Ende der angegebenen Laufzeit haltbar. Die Laufzeit ist dem Reagenzienetikett zu entnehmen. Bei beschädigten Fläschchen darf das Produkt nicht mehr verwendet werden. Warnhinweis und Vorsichtsmaßregeln Eine Trübung oder andere sichtbare Veränderungen des Produktes können ein Hinweis auf bakterielle Verunreinigung sein. In diesem Fall darf das Produkt nicht eingesetzt werden, da die Ursache der Veränderung durch den Hersteller geklärt werden muss. Alle Techniken sind nach den Vorgaben des Herstellers anzuwenden. Jede Abweichung von den Vorgaben des Herstellers liegt in der Verantwortung des Anwenders. Abgearbeitete Tests müssen als Abfall der Gruppe B wie beschrieben im „Merkblatt über die Vermeidung und die Entsorgung von Abfällen aus öffentlichen und privaten Einrichtungen des Gesundheitsdienstes“ und der „Richtlinie für Krankenhaushygiene und Infektionsprävention“, entsorgt werden. Durch die biotechnologische Herstellung der humanen monoklonalen Antikörper ist das Risiko einer Kontamination durch infektiöse Krankheitserreger nahezu ausgeschlossen. Dennoch sollten alle Testreagenzien biologischen Ursprungs als potentielle Überträger von Hepatitis, HIV oder anderen infektiösen Krankheitserregern behandelt werden. Angemessene Sicherheitsvorkehrungen werden empfohlen. Regelmäßige interne Qualitätskontrollen sollten gemäß den jeweiligen nationalen Richtlinien (1), durchgeführt werden. Principe du test Le principe du test est basé sur l´hémagglutination qui décèle l´antigène correspondant des érythrocytes. Le groupe sanguin s est défini par la présence ou l´absence des caractéristiques s sur les érythrocytes. L´antigène s réagit de façon spécifique aux anticorps correspondants de Seraclone® Anti-s (MNS4). L´agglutination met en évidence une réaction positive. Le réactif peut être utilisé pour le test en tube. Réactif Le composant actif du réactif est issu de la culture in-vitro d’une lignée d’hybridome humain qui secrète un anti s IgG. Ils présentent la spécificité et la reproductibilité constante caractéristique des anticorps monoclonaux. Seraclone® Anti-s (MNS4) clone P3YAN3 Réactifs supplémentaires et ustensiles nécessaires • Pipettes (volume par goutte 40-50µl) • Eau destillée • Solution saline isotonique • Antiglobuline humaine (par ex. Anti-Human-Globulin Color [REF] 804115 et 804130) • MLB 2 ([REF] 805 205, 805 200) • Erythrocytes de test sensibilisés à l´IgG Coombscell-E ([REF] 816 030) • Tubes de verre • Centrifugeuse de laboratoire Echantillons Les échantillons sanguins sont prélevés selon des méthodes généralement reconnues. Utiliser des échantillons frais, non hémolytiques (sang natif, EDTA ou citraté). Dans la mesure où la conduite du mode opératoire nécessite une suspension d´érythrocytes, celle-ci est confectionnée à partir des érythrocytes de l'échantillon de sang centrifugé (par ex. 2 min. à 800 - 1000 x g) avec une solution saline isotonique comme milieu de suspension. Avant de préparer la suspension cellulaire, il est conseillé de laver les érythrocytes de test à 2 reprises, voire autant de fois qu'il sera nécessaire pour obtenir la limpidité du surnageant. Les échantillons fortement lipémiques, ictériques ou présentant une contamination microbienne peuvent conduire à des résultats de tests non fiables. Mode opératoire Test en tube 1. Mettre en suspension les érythrocytes à examiner dans une solution saline isotonique de 3 à 5%. 2. Mélanger respectivement dans une tube adéquatement marquée 1 goutte de réactif et 1 goutte de suspension d´érythrocytes. 3. Incuber pendant 30 minutes à 37°C. 4. Laver les érythrocytes 3 fois avec une solution saline isotonique. Décanter ensuite intégralement le surnageant. 5. Ajouter au sédiment 2 gouttes d´antiglobuline humaine et mélanger. 6. Centrifuger pendant 20 sec à 800 - 1000 x g. 7. Observer l´agglutination en agitant doucement. Test en tube, methode en LISS 1. Mettre en suspension les érythrocytes à examiner dans une solution saline isotonique de 3 à 5%. 2. Mélanger respectivement dans une tube adéquatement marqué 1 goutte de réactif, 1 goutte de suspension d´érythrocytes et 2 gouttes de MLB2 . 3. Incuber pendant 10 minutes à 37°C. 4. Laver les érythrocytes 3 fois avec une solution saline isotonique. Décanter ensuite intégralement le surnageant. 5. Ajouter au sédiment 2 gouttes d´antiglobuline humaine et mélanger. 6. Centrifuger pendant 20 sec à 800 - 1000 x g. 7. Observer l´agglutination en agitant doucement. Il convient d´inclure à chaque test un contrôle positif et négatif (1). Interprétation des résultats L´évaluation des forces de réaction s´opère conformément aux dispositions du Technical Manual (2): Force de Agglutination réaction 4+ Un agglutinat unique. Aucun érythrocyte libre. 3+ Réaction forte. Quelques agglutinats de grande dimension. 2+ Agglutinats de moyenne dimension, aucun érythrocyte libre. 1+ Nombreux agglutinats de faible dimension sur un fond d´érythrocytes libres. +/Peu d´agglutinats macroscopiquement reconnaissables à l´intérieur de la suspension d´érythrocytes. Nombreux agglutinats reconnaissables au microscope. Suspension d´érythrocytes homogène sans agglutinats reconnaissables. 2 ml [PRES] Conservé avec: < 0,1 % de NaN3 [ACT] [IVD] Réactif pour usage diagnostique in vitro Ne doit être utilisé que par le personnel de laboratoire formé à cet effet. Le [ACT] figure sur l´étiquette. Les symboles utilisés sont conformes à la norme EN 980. Les symboles non visés dans la norme EN 980 sont expliqués dans le texte. Si ce contrôle donne un résultat | 186988/03 – 08/2012 négatif, ceci peut être dû à une erreur dans le mode opératoire ou bien à une perte de réactivité du réactif. En cas de résultats douteux dont la cause ne serait pas évidente, veuillez consulter le Bio-Rad-Service (Tél. +49-6103-3130-611) qui se tient à votre disposition. Stabilité Une fois le flacon ouvert, le réactif peut être conservé dans des conditions adéquates (2...8°C) jusqu´à la date de péremption indiquée. La durée de stabilité figure sur l´étiquette du réactif. Si le flacon est endommagé, le produit ne doit plus être utilisé. Avertissements et précautions Tout aspect turbide ou d'autres changements visibles du produit peuvent indiquer une contamination bactérienne. Dans ce cas, ne pas utiliser le produit, car le fabricant est dans l´obligation d´éclaircir la cause du changement de celuici. Toutes les techniques doivent être appliquées conformément aux instructions du fabricant. Toute dérogation aux instructions du fabricant relève de la responsabilité de l´utilisateur. Les matériaux de tests utilisés doivent être évacués en application des directives nationales respectives. Grâce à la production des anticorps monoclonaux humains par des procédés biotechnologiques, le risque de contamination par germes pathogènes infectieux est pratiquement exclu. Cependant, tous les réactifs de test d´origine biologique doivent être considérés comme étant potentiellement porteurs des virus de l´hépatite, du HIV ou d´autres germes pathogènes. Il est conseillé de prendre les mesures de sécurité adéquates. Il est recommandé d´effectuer en interne, au minimum un contrôle positif et un contrôle négatif conformément aux directives nationales, par ex. (1). Finalidad prevista Seraclone® Anti-s (MNS4) se emplea para la detección de la propiedad del grupo sanguíneo s. Principio del test El test se basa en el principio del test de hemaglutinación para la detección del correspondiente antígeno eritrocitario. El grupo sanguíneo s es definido por la presencia o ausencia de la propiedad s sobre los eritrocitos. El antígeno s reacciona de manera específica con el correspondiente anticuerpo contenido en Seraclone® Anti-s (MNS4). La reacción positiva es visible si se produce una aglutinación. Este reactivo puede ser utilizado para el test en tubo de ensayo. Reactivo Seraclone® Anti-s (MNS4) contiene como componente reactivo anticuerpos monoclonales de la clase de inmunoglobulina IgG de origen humano. Los anticuerpos proceden del sobrenadante del cultivo celular y presentan la especificidad y reproducibilidad características de los anticuerpos monoclonales. Seraclone® Anti-s (MNS4) clon P3YAN3 Reactivos y materiales necesarios que no se suministran • Pipetas (volumen de dispensación: 40-50μl) • Agua destilada • Solución salina isotónica • Antiglobulina humana (p.ej. Anti-Human-Globulin Color [REF] 804115 y 804130) • MLB 2 ([REF], 805 205, 805 200) • Eritrocitos de ensayo sensibilizados con IgG Coombscell-E ([REF] 816 030) • Tubos de ensayo de vidrio • Centrifugadora de laboratorio Muestras Las muestras de sangre se obtendrán según los métodos generalmente válidos. Se deben utilizar muestras frescas, que no presenten hemólisis (sangre nativa, con EDTA o con citrato). Si para la ejecución del análisis se requiere una suspensión eritrocitaria, ésta se preparará de los eritrocitos de la muestra de sangre centrifugada (p. ej. 2 min. a 800 - 1000 x g) con solución salina isotónica como medio de suspensión. Antes de preparar la suspensión celular, se recomienda lavar los eritrocitos a analizar por lo menos 2 veces o hasta obtener un sobrenadante limpio. Las muestras extremadamente lipémicas, ictéricas o contaminadas microbiológicamente pueden dar lugar a resultados poco fiables. Procedimiento Test en tubo de ensayo 1. Suspender los eritrocitos que de desean analizar al 3-5% en solución salina isotónica. 2. En un tubo adecuadamente rotulado verter 1 gota de reactivo y 1 gota de la suspensión eritrocitaria. 3. Incubar durante 30 minutos a 37°C. 4. Lavar los eritrocitos 3 veces con solución salina isotónica. Decantar el sobrenadante completamente. 5. Añadir 2 gotas de antiglobulina humana al sedimento y mezclar. 6. Centrifugar 20 segundos a 800 - 1000 x g. 7. Verificar la aglutinación agitando suavemente. Test en tubo de ensayo con LISS 1. Suspender los eritrocitos que de desean analizar al 3-5% en solución salina isotónica. 2. En un tubo adecuadamente rotulado verter 1 gota de reactivo, 1 gota de la suspensión eritrocitaria y 2 gotas de MLB 2. 3. Incubar durante 10 minutos a 37°C. 4. Lavar los eritrocitos 3 veces con solución salina isotónica. Decantar el sobrenadante completamente. 5. Añadir 2 gotas de antiglobulina humana al sedimento y mezclar. 6. Centrifugar 20 segundos a 800 - 1000 x g. 7. Verificar la aglutinación agitando suavemente. Para todos los análisis se deberá efectuar paralelamente un control negativo y uno positivo (1). Interpretación de los resultados La intensidad de reacción se evaluará de acuerdo con lo dispuesto en el Technical Manual (2): Intensidad de Aglutinación reacción 4+ Un solo aglutinado. Ausencia de eritrocitos libres. 3+ Fuerte reacción. Varios aglutinados grandes. 2+ Aglutinados medianos; ausencia de eritrocitos libres. 1+ Numerosos aglutinados pequeños sobre un fondo de eritrocitos libres. +/Unos pocos aglutinados visibles macroscópicamente en el interior de la suspensión de eritrocitos. Numerosos aglutinados visibles microscópicamente. Suspensión eritrocitaria homogénea, sin aglutinados perceptibles. Características de funcionamiento y limitaciones del método Los resultados negativos en el test indirecto de antiglobulina humana deben ser verificados con células de ensayo cubiertas de anticuerpos. Un resultado negativo en este control puede ser debido a un error en la ejecución del test o a una disminución de la reactividad del reactivo. Estabilidad Una vez abierto el reactivo, el producto puede ser conservado en condiciones adecuadas (2...8°C) hasta la fecha de caducidad que se indica. La fecha de caducidad viene indicada en la etiqueta del reactivo. No utilizar el producto si el vial está dañado. Advertencias y precauciones Un enturbiamiento u otra alteración visible del producto puede ser indicio de contaminación bacterial. En este caso no se debe utilizar el reactivo, ya que la causa del alteración debe ser esclarecida por el fabricante. Todas las técnicas se han de realizar de acuerdo con las instrucciones del fabricante. El usuario es responsable de cualquier divergencia con respecto a tales instrucciones. El material de ensayo ya usado debe ser eliminado de acuerdo con las normativas nacionales relativas a la eliminación de residuos infecciosos. Gracias a la producción biotecnológica de los anticuerpos monoclonales humanos, el riesgo de contaminación por agentes infecciosos queda prácticamente descartado. No obstante, todos los reactivos de ensayo de origen biológico deberán ser considerados como potenciales portadores de hepatitis, HIV u otros agentes patógenos infecciosos. Se recomienda adoptar las precauciones adecuadas. Se deberán efectuar controles internos de calidad regularmente en conformidad con las normas nacionales vigentes, p.ej. (1). Finalità d'uso Seraclone® Anti-s (MNS4) viene impiegato per la rilevazione dell’antigene eritrocitario s. Principio del test Il test si basa sul principio di emoagglutinazione per individuare l'antigene eritrocitario corrispondente. Il gruppo sanguigno s si definisce tramite la presenza o l'assenza dell’antigene s sugli eritrociti. L'antigene s reagisce in modo specifico con l'anticorpo corrispondente in Seraclone® Anti-s (MNS4). La reazione positiva è visibile se avviene l'agglutinazione. Il reagente viene utilizzato per test in provetta. Reagente Seraclone® Anti-s (MNS4) contiene come componente reattivo anticorpi monoclonali di classe immunoglobulinica IgG di origine umana. Deriva dal surnatante di colture cellulari e presenta la specificità e la riproducibilità costanti caratteristiche degli anticorpi monclonali. Seraclone® Anti-s (MNS4) clone P3YAN3 Reagenti supplementari e materiali necessari • Pipette (volume di dispensazione: 40-50 μl) • Acqua distillata • Soluzione salina isotonica • Antiglobuline umane (ad es. Coombscell-E [REF] 816 030) • Provette in vetro • Centrifuga da laboratorio Stabilità Una volta aperto il flacone, il prodotto può essere conservato in condizioni appropriate (2...8°C) fino alla data di scadenza indicata. La data di scadenza è riportata sull'etichetta del reagente. Se i flaconi dovessero risultare danneggiati, il prodotto non va più utilizzato. Avvertenze e precauzioni L’eventuale presenza di torbidità o altri cambiamenti visibili nel prodotto possono essere indice di contaminazione batterica. In tal caso il prodotto non va utilizzato in quanto spetta al produttore chiarire la causa della cambiamenti. Tutte le tecniche vanno impiegate secondo le istruzioni del produttore. L’utilizzatore è responsabile di qualsiasi deviazione da tali istruzioni. I materiali dei test utilizzati vanno smaltiti conformemente alle rispettive normative nazionali vigenti. Grazie alla produzione biotecnologica degli anticorpi monoclonali umani, il rischio di contaminazione con agenti patogeni infettivi può essere quasi escluso. Tuttavia, tutti i reagenti di origine biologica devono essere considerati come potenziali portatori di epatite, HIV o altri agenti patogeni infettivi. Si consigliano adeguate misure precauzionali. Controlli di qualità interni vanno condotti regolarmente in conformità alle rispettive normative vigenti, ad es. (1). Dovrebbero essere utilizzati almeno un controllo positivo e uno negativo. Devono essere impiegati campioni freschi, che non presentino emolisi (sangue nativo, con EDTA o citrato). Se per il procedimento è necessaria una sospensione eritrocitaria, questa viene preparata dagli eritrociti del campione di sangue centrifugato (ad es. 2 minuti a 800 - 1000 x g) con soluzione salina isotonica quale mezzo di sospensione. Si raccomanda di lavare gli eritrociti in esame prima della preparazione della sospensione cellulare per almeno due volte o fino ad ottenere un surnatante limpido. Campioni fortemente lipemici, itterici o con contaminazione microbica possono dare risultati falsati. Esecuzione del test Test in provetta 1. Gli eritrociti in esame vanno sospesi al 3-5% in soluzione salina isotonica. 2. Dispensare in una provetta opportunamente contrassegnata 1 goccia di reagente e 1 goccia di sospensione eritrocitaria. 3. Incubare per 30 minuti ad una temperatura di 37°C. 4. Lavare gli eritrociti 3 volte con soluzione salina isotonica. Decantare completamente il surnatante. 5. Aggiungere 2 gocce di antiglobuline umane al sedimento e miscelare. 6. Centrifugare per 20 secondi a 800 - 1000 x g. 7. Verificare l'agglutinazione scuotendo leggermente. Test in provetta, metodo in LISS 1. Gli eritrociti in esame vanno sospesi al 3-5% in soluzione salina isotonica. 2. Dispensare in una provetta opportunamente contrassegnata 1 goccia di reagente, 1 goccia di sospensione eritrocitaria e 2 gocce di MLB 2. 3. Incubare per 10 minuti ad una temperatura di 37°C. 4. Lavare gli eritrociti 3 volte con soluzione salina isotonica. Decantare completamente il surnatante. 5. Aggiungere 2 gocce di antiglobuline umane al sedimento e miscelare. 6. Centrifugare per 20 secondi a 800 - 1000 x g. 7. Verificare l'agglutinazione scuotendo leggermente. Per ciascuna analisi vanno effettuati un controllo positivo e un controllo negativo (1). Interpretazione dei risultati La valutazione della forza di reazione viene eseguita secondo le istruzioni riportate nel Technical Manual (2): Forza di Agglutinazione reazione 4+ Un unico agglutinato. Nessun eritrocita libero. 3+ Reazione forte. Alcuni agglutinati di grandi dimensioni. 2+ Agglutinati di dimensioni intermedie; nessun eritrocita libero. 1+ Numerosi agglutinati di piccole dimensioni su uno sfondo di eritrociti liberi. +/Pochi agglutinati rilevabili macroscopicamente all'interno della sospensione di eritrociti. Numerosi agglutinati riconoscibili al microscopio. Sospensione eritrocitaria omogenea, senza agglutinati riconoscibili. Caratteristiche delle prestazioni e limiti del metodo Risultati negativi nel test di Coombs indiretto vanno verificati eseguendo un test con emazie rivestite di anticorpi. Un risultato negativo in questo controllo può essere dovuto ad un errore nell'esecuzione del test o ad una diminuzione di reattività del reagente.
